主要技术内容:尿毒症是各种慢性肾病的最终结果。某些急性肾脏病或急性肾损伤患者的肾功能也不能完全恢复，直接进入尿毒症需要肾脏替代治疗或遗留慢性肾脏病，最终仍逐渐进展致尿毒症。肾移植、血液透析和腹膜透析是成功的肾脏替代治疗方式，尿毒症患者可依赖肾脏替代治疗长期存活。由于肾源的匮乏，尿毒症患者依赖血液透析或腹膜透析等待合适的肾源，或因病情不适合肾移植则依赖血液透析或腹膜透析长期存活。腹膜透析是一种典型的居家血液净化治疗模式，患者只在预定好的时间定期到血液净化室或腹膜透析随访点进行随访以对病情进行评估并调整腹膜透析方案，因此有大量的自由时间。由于对操作技术要求较高，目前，国际上血液透析患者大多在血液净化室接受治疗，只有少数患者居家治疗。与居家腹膜透析一样，居家血液透析患者可获得更多的自由时间，社会回归率高于在血液净化室接受治疗的患者；成规模的居家血液透析也会大量节省医疗资源和人力资源；居家血液透析比在血液净化室的治疗更容易实现低效长时等更优的血液净化模式；在传染病流行期间，居家治疗可减少外出，降低受染机会。目前，针对居家腹膜透析治疗已经有大量出自行业协会的指南性意见，但针对居家血液透析治疗，目前尚无指南性意见。这阻碍了有提供这种服务意向的血液净化室迈出第一步。目前，我国仅有个位数患者在接受真正意义上的居家血液透析，经验十分匮乏。国际上也未见到类似标准或行业协会发布的临床实践指南。在撰写本标准时，参考了提供或参与居家血液透析服务的国外专家在正式刊物上公开发布的经验。我国居家血液透析目前处于起步阶段，撰写本标准的目的是为借鉴国外经验，以达到在我国开展居家血液透析的道路中少走弯路。随着我国居家血液透析的逐渐增多和普及，本标准将不断完善和更新
